An Aging Life Care Professional is educated and experienced in several fields related to Aging Life Care management, including, but not limited to counseling, gerontology, mental health, nursing, occupational therapy, physical therapy, psychology, or social work; with a specialized focus on issues related to aging and eldercare.

Aging Life Care Management is a client-centered approach to caring for older adults or others facing ongoing health challenges. Shannon Conlin

Team Member 

“My name is Shannon Conlin.  I work with Diversified Nurse Consultants as a Case Manager for  Private Clients and as a Case Manager for Anthem Ground Game Health. I have been practicing Medical Social Work and Case Management for over 25 years.  I have been employed with  Diversified Nurse Consultants for 7 1/2 years.   I hold a Masters Degree in Social Work from the University of Louisville and a Bachelors degree from Spalding University.  I have a current license in the State of Kentucky. 

My passion is caring for our aging population,  providing the best care and resources to help them navigate through our complex health care system.”

Pam Ross

Team Member 

Pam is a RN, BSN who began working with Diversified Nursing Consultants in 2014.  She comes to us with a varied nursing background in oncology, acute cardiac care, emergency and occupational nursing, Informatics and case managment.  She began her career with DNC as a care manager, transferring into the role of Clinical Supervisor, responsible for education and oversite of field care managers, as well as overall management of the Care Management program.   Responsibilities include intake of clients including identifying initial client program needs, employee training and orientation and ongoing care staff supervision.  

Pam is also Director of Peggy’s Place Adult Life Center. Peggy’s Place is a medical and social model Day Center serving the medical and social needs of our participants.  She provides supervision and oversite to the Center, as well as enjoys one on one time with many of those in attendance.

Barbara Wheatley

Team Member 

I graduated from the University of Louisville School of Nursing in 1994 with a Bachelor’s of Science in Nursing Degree.  I started my career working in the hospital on various medical-surgical and cardiac units.  After spending several years in the hospital setting, I transitioned to teaching patients about their medical conditions and how to stay healthy by working as a nurse in a telephonic education position.  In 2003, I obtained by Certification in Case Management (CCM) from the Commision for Case Manager Certification. I began working as the case manager for a brain injury rehabilitation center serving clients with acquired brain injuries. In 2014, I joined Diversified Nurse Consultants as a case manager. I am lucky to serve as an advocate for my clients, especially those with dementia.  Since 2016 I have been a member of the Aging Life Care Association.  I am licensed in both Kentucky and Indiana.

Tracy Book Diers

Past – CEO I President/ Co-Founder I RN, BSN, LNC, CPPN

No longer with Diversified Nurse Consultants, LLC, Tracy holds a professional registered nursing license in both Indiana and Kentucky. She is a past President and Co-Founder of Peggy’s Place Adult Life Center, and is a past Board Member of the Indiana Association of Adult Services, as well as, an active member of the National Adult Day Services Association.
